How do I make an appointment?
In order to make or change an appointment, please contact the office location where you are going to be seen. Dunkirk and Westfield Clinic –(716) 366-7150, Irving (716) 934-3493.
What do I need to bring to my first appointment?
If this will be your first time being seen by one of our physicians, make sure that you bring your insurance card, a list of medications you are currently taking, a list of allergies you may have, a brief medical history for our records, any test results, x-rays, or MRI’s and a referral number if you are a member of an HMO.
How do I cancel an appointment?
If you need to cancel, please give us 24 HOUR notice by telephone, and please call the office in which you where going to be seen for your appointment. If your call is after hours, please leave a message with our answering service with your name, time and date of the appointment and if you wish to reschedule.
What insurance carriers do you participate with?
Lakeshore Orthopedic Group, P.C. participates with several HMO’s, insurance providers, and Government programs. Please call your carrier to verify their participation with our group.
What are your office hours?
Our office hours vary by location which on listed on our main page “About Us” . These office hours do not reflect the physicians which vary pending upon their surgery schedule.
How do I get my prescription refilled?
Please contact our office by telephone, Dunkirk and Westfield (716) 366-7150, Irving (716) 934-3493, before your prescription runs out. Out office does not provide this service on the weekends.
When do I pay for my visit?
Payment is appreciated at the time of visit, if a payment or co-pay is required. Please make sure you bring your insurance information with you. Payment can be in the form of cash, personal check or credit card (Dunkirk and Irving only at this time). We accept Mastercard and Visa.
What should I bring for my first visit for a worker’s compensation or no-fault case?
Please bring your insurance carrier’s name and address, the date of your injury, your employer’s name and address, your worker’s compensation number or no-fault policy number, and any test results, x-rays, or MRI’s for any worker’s compensation or no-fault initial visit.
How do I get my disability form filled out?
If you have a disability form that needs to be completed, please mail or drop off the form at our office where you have been seen. There is a $7.00 charge/per form payable by cash, check or credit card (most major cards accepted,to Lakeshore Orthopedic Group, P.C., when you mail or drop off the form. Payment is required before the form will be filled out.
How do I re-schedule my surgery if needed?
If you have to re-schedule a surgery, please call the office where you have been seen: Dunkirk and Westfield (716) 366-7150, Irving (716) 934-3493 to have it re-scheduled.
Where does my physician perform my surgery?
Our physicians each perform their surgeries at the following hospitals:
Brooks Memorial Hospital- Dr. Cardamone, Dr. Fitzgerald, Dr. Ouweleen
Lakeshore Hospital (TLC)- Dr. Cardamone, Dr. Ouweleen
Westfield Memorial Hospital- Dr. Fitzgerald

Your Health Care Proxy
The New York Health Care Proxy Law allows you to appoint someone you trust — for example, a family member or close friend — to make health care decisions for you if you lose the ability to make decisions yourself. By appointing a health care agent, you can make sure that health care providers follow your wishes. Your agent can also decide how your wishes apply as your medical condition changes. Hospitals, doctors and other health care providers must follow your agent’s decisions as if they were your own. You may give the person you select as your health care agent as little or as much authority as you want. You may allow your agent to make all health care decisions or only certain ones. You may also give your agent instructions that he or she has to follow. This form can also be used to document your wishes or instructions with regard to organ and/or tissue donation.
