How do I make an appointment

In order to make or change an appointment, please contact the office location where you are going to be seen.

What do I need to bring to my first appointment?

If this will be your first time being seen by one of our physicians, make sure that you bring:

  • your insurance card
  • a list of medications you are currently taking
  • a list of allergies you may have
  • a brief medical history for our records
  • any test results, x-rays, or MRI’s
  • a referral number if you are a member of an HMO
  • completed copies of our New Patient Forms (download now)

How do I cancel an appointment?

If you need to cancel, please give us 24 HOUR notice by telephone, and please call the office in which you where going to be seen for your appointment. If your call is after hours, please leave a message with our answering service with your name, time and date of the appointment and if you wish to reschedule.

What insurance carriers does our Group participate with?

Lakeshore Orthopedic Group, P.C. participates with several HMO’s, insurance providers, and Government programs. Please call your carrier to verify their participation with our group.

What are your office hours?

Our office hours vary by location which on listed on our main page “About Us” . These office hours do not reflect the physicians which vary pending upon their surgery schedule.

How do I get my prescription refilled?

Please contact our office by telephone before your prescription runs out. Our office does not provide this service on the weekends.

When do I pay for my visit?

If a payment or co-pay is required, payment is appreciated at the time of your visit. Please make sure to bring your insurance information with you. Payment can be in the form of cash, personal check or credit card (Dunkirk and Irving only at this time). We accept Mastercard and Visa.

What should I bring for a worker's comp or no-fault case?

Please bring:

  • your insurance carrier’s name and address
  • the date of your injury
  • your employer’s name and address
  • your worker’s compensation number or no-fault policy number
  • any test results, x-rays, or MRI’s
  • completed copies of our New Patient Forms (download now)

How do I get my disability form filled out?

If you have a disability form that needs to be completed. Please mail the form, along with payment, to us (click here for address) or drop off the form and payment at our office where you have been seen. Payment is required prior to the form being filled out

There is a $7.00 charge/per form payable by cash, check or credit card (most major cards accepted).

How do I re-schedule my surgery if needed?

If you have to re-schedule a surgery, please call the office where you have been seen to have it re-scheduled.
